CHS provides a range of primary medical care services to fit your needs, as well as the needs of your whole family. Same day appointments and urgent care are also available.

ICHS sliding-scale fees are based on family size and income. Patients will be asked to complete an application to determine how much discount they qualify for. 

Hours:

 Medical:

 Monday and Wednesday, 7:30 am - 6:00 pm.

 Tuesday, Thursday and Friday, 8:30 am - 5:00 pm.

Dental:

 Monday - Saturday, 7:30 am - 6:00 pm.
