CHAP is a nonprofit community health center providing medical, dental and behavioral health services to children and adults. We serve all the foothill communities of the San Gabriel Valley and greater Los Angeles county with three clinic locations. New patients are welcome; we invite you to make CHAP your medical home.

We provide services to insured and uninsured residents of the San Gabriel Valley, but any resident of Los Angeles County is eligible for services. No one is declined service due to inability to pay.

CHAP's mission is to ensure that high quality health care services are available and affordable to those who need them. No one is turned away due to inability to pay. Low and no cost services are available to those who meet income qualifications.

CHAP also welcomes patients who qualify for one of the following coverage programs.

- Healthy Families
