Free Specialty Health services nonprofit for Polk County residents.

We Care serves Polk County residents who have no insurance and who are at or below 200% of the federal poverty guidelines, ensure access to specialty medical care for low income, uninsured residents of Polk County.

They are not eligible for any source of funded medical care, are desperately in need of specialty medical care and have no where to turn for help.

PATIENT MUST BE REFERRED BY A PRIMARY PHYSICIAN. If the caller does not have a primary, they can be referred by LVIM or the Health Department.

For Households with more than 8 persons, add $4,480 for each additional person.
*Alaska and Hawaii have different rates for HUD federal poverty guidelines.
These numbers above represent 100% of the Federal Poverty Rate. In order to get reduced or free services from some clinics, they use a sliding fee scale based on your income.When they use a sliding fee scale, the 100% rate can be different than 100%. In those cases, using for example a 200% federal poverty level, you will only need double the 100% number listed above to 200%.

    I have been taking care of my family member who was diagnosed with type 2 diabetes a week ago after been in icu and then into a regular room . he cant pay the 350.00 for his meds and we are having a hard time keeping his sugar lower then 2p0.
