If your health plan is not listed we will work with you and your insurance company to establish care at FoundCare for a medical home.
If you are uninsured and would like to establish a medical home at FoundCare's Health Center we accept appointments which include laboratory services for a nominal out of pocket fee. All patients receive the same high-quality standard of care regardless of whether or not they have insurance. Dental Care: Did you know that good oral health has been linked to good overall health? FoundCare's Health Center offers comprehensive, preventive, restorative and emergency dental care in a warm and friendly setting.
Comprehensive dentistry includes exams, x-rays and treatment planning.
preventive dentistry includes cleanings, fluoride and sealants.
Restorative dentistry includes fillings and crowns.
Emergency dental treatment includes toothaches, oral sores, and broken teeth.
The Health Center uses a sliding fee scale based on income to assess patient charges. We also accept the following insurance plans:
Dental: MCNA, DentaQuest, Delta Dental, Cigna, Metlife and Guardian.

For Households with more than 8 persons, add $4,480 for each additional person.
*Alaska and Hawaii have different rates for HUD federal poverty guidelines.
These numbers above represent 100% of the Federal Poverty Rate. In order to get reduced or free services from some clinics, they use a sliding fee scale based on your income.When they use a sliding fee scale, the 100% rate can be different than 100%. In those cases, using for example a 200% federal poverty level, you will only need double the 100% number listed above to 200%.
