COVID-19 (Coronavirus) Information for Patients
Contact your healthcare provider by phone before coming to a clinic or hospital if you meet the following criteria:
Symptoms such as fever, cough or shortness of breath and
Travel to countries with coronavirus alerts within 14 days of onset of symptoms or
Contact with someone with confirmed COVID-19 within 14 days of onset of systems
This clinic operates under a SLIDING SCALE model. This means that it MAY NOT be free depending on your income. You will be required to prove financial need in order to receive free services or services at a reduced cost. This is a health care center funded by the federal government. This means even if you have no insurance you can be covered. The center is also income based for those making an income. This health center can cover services such as checkups, treatment, pregnancy care (where applicable), immunizations and child care (where applicable), prescription medicine and mental and substance abuse where applicable. ACCESS provides a national model of quality, affordable community health care for medically underserved populations that are tailored to meet the specific health needs of the individuals, families and communities we serve.
Cost: Medicaid, Medicaid HMO, Medicare and most major health plans are accepted to cover the costs of medical care. A sliding fee scale based on ability to pay is available for uninsured patients. Case management services are provided at no charge.

All patients are welcome at ACCESS regardless of if you have insurance. Patients without the ability to pay will not be denied services. When visiting the health center, a staff member will be happy to help you enroll in any benefits you may be eligible to receive. We accept All Kids, CountyCare, Family Care, Family Health Network, Harmony, Blue Community „ , Medicare, Medicaid and most health insurance plans, including several managed care plan options. If you are uninsured, you may be eligible for plans available throughÂ GetCoveredIllinois.Gov, the Illinois health insurance marketplace (please see below for more information).
Sliding Fee Scale Program
We also offer a sliding fee scale program to those who are uninsured or underinsured and low income. Any payment that is required for your visit is due at the time of you r appointment.
