|Jun 5, 2017||$20||Completely Free Clinic||Had surgery on my neck which was noted as a C5 had surgery at Jack Hughston in Phenix City, Alabama and received a charity scholarship for Shepherd Spine Center in Atlanta. I was discharged on Thursday, June 1st and is in need of a physician. Before the accident I had just started my job and insurance was not active at the time. Social Security has a open claim for me for disability which is pending. I need to have a follow up done and I don't have the funds to pay a physician since I don't have health insurance.|
