Contact your healthcare provider by phone before coming to a clinic or hospital if you meet the following criteria:
Symptoms such as fever, cough or shortness of breath, fatigue, headache, muscle or body aches, loss of taste or smell, sore throat, congestion or runny nose.
Contact with someone with confirmed COVID-19 within 14 days of onset of systems
This clinic operates under a SLIDING SCALE model. This means that it MAY NOT be free depending on your income. |Jun 5, 2017||$20||Completely Free Clinic||Had surgery on my neck which was noted as a C5 had surgery at Jack Hughston in Phenix City, Alabama and received a charity scholarship for Shepherd Spine Center in Atlanta. I was discharged on Thursday, June 1st and is in need of a physician. Before the accident I had just started my job and insurance was not active at the time. Social Security has a open claim for me for disability which is pending. I need to have a follow up done and I don't have the funds to pay a physician since I don't have health insurance.|
