Eligibility:

* Medical Care

Must reside in DeKalb or Noble Counties
Be Uninsured
Be Underinsured (out-of-pocket costs of insurance available to you represent 10% or more of your income)
Be at or below 200% of the Federal Poverty Guidelines

*Dental Care:

Must reside in DeKalb or Noble Counties
Must be 18 years of age or older
Have no dental insurance or benefits
Be at or below 200% of the Federal Poverty Guidelines

Medical Clinic Hours:

Mondays     9am-12pm / 1pm-4pm     Walk-in
Tuesdays     9am-12pm     Appointments / 1pm-3pm     Appointments
Wednesdays     9am-1pm     Walk-in
Thursdays 4pm-8pm     Walk-in
Fridays     Closed
